1. From the Patient HomePage login screen, click the “Forgot User ID?” link.

  2. In the new window, enter your Email ID in the field and click send verification code.

Important Note: The Email ID has to be the Email ID the account was registered under!

Warning: Do not close this window until you have completed through Step 4!

  1. You will receive a verification code in your email inbox. Enter that verification code into the Verification Code field in Step 2. 

  2. Upon successful entry of the verification code, you will be able to enter the new User ID and confirm the new User ID.
