- From the Patient HomePage login screen, click the “Forgot User ID?” link.
- In the new window, enter your Email ID in the field and click send verification code.
Important Note: The Email ID has to be the Email ID the account was registered under!
Warning: Do not close this window until you have completed through Step 4!
- You will receive a verification code in your email inbox. Enter that verification code into the Verification Code field in Step 2.
- Upon successful entry of the verification code, you will be able to enter the new User ID and confirm the new User ID.
